Biomarker predicts risk of breast cancer recurrence after tamoxifen treatment

June 29, 2013 ? A biomarker reflecting expression levels of two genes in tumor tissue may be able to predict which women treated for estrogen-receptor (ER)-positive breast cancer should receive a second estrogen-blocking medication after completing tamoxifen treatment. In their report being published online in the Journal of the National Cancer Institute, Massachusetts General Hospital (MGH) Cancer Center investigators describe finding that the HOXB13/IL17BR ratio can indicate which women are at risk for cancer recurrence after tamoxifen and which are most likely to benefit from continuing treatment with the aromatase inhibitor letrozole (Femara).

"Most patients with early-stage, ER-positive breast cancer remain cancer-free after five years of tamoxifen treatment, but they remain at risk of recurrence for 15 years or longer after their initial treatment," says Dennis Sgroi, MD, of the MGH Cancer Center and Department of Pathology, lead and corresponding author of the report. "Our biomarker identifies the subgroup of patients who continue to be at risk of recurrence after tamoxifen treatment and who will benefit from extended therapy with letrozole, which should allow many women to avoid unnecessary extended treatment."

Previous research by Sgroi's team, in collaboration with investigators from bioTheranostics Inc., discovered that the ratio between levels of expression of two genes -- HOXB13 and IL17BR -- in tumor tissue predicted the risk of recurrence of ER-positive, lymph-node-negative breast cancer, whether or not the patient was treated with tamoxifen. The current study of patients from MA.17, the highly successful clinical trial of letrozole, was designed to evaluate the usefulness of the HOXB13/IL17BR ratio for both prognosis -- predicting which tamoxifen-treated remained patients at risk of recurrence -- and for identifying who could benefit from continued treatment with letrozole.

To answer those questions the investigators analyzed primary tumor samples and patient data from the placebo-controlled MA.17 trial, which confirmed the ability of extended letrozole therapy to improve survival after the completion of tamoxifen treatment. Tissue samples were available from 83 patients whose tumors recurred during the study period -- 31 who had received letrozole and 52 in the placebo group -- and 166 patients with no recurrence, 91 of whom had received letrozole, with 75 getting the placebo. Analysis of the tumor samples revealed that a high HOXB13/IL17BR ratio -- meaning the expression level of HOXB13 is greater than that of IL17BR -- predicts an increased risk for tumor recurrence after tamoxifen therapy, but that elevated risk drops significantly if a patient receives letrozole

Paul E. Goss, MD, PhD, director of the Breast Cancer Research Program at the MGH Cancer Center and a co-author of the report, explains, "This discovery means that about 60 percent of women with the most common kind of breast cancer can be spared unnecessary treatment with the concommitant side effects and costs. But more importantly, the 40 percent of patients who are at risk of recurrence can now be identified as needing continued therapy with letrozole, and many will be spared death from breast cancer." He and Sgroi note that their findings need to be validated by additional studies before they can be put into clinical practice.

BlueStacks introduces the GamePop Mini, its first subscription-based 'free' game console

In an effort to outdo itself, BlueStacks is announcing the GamePop Mini for the cube-buying averse. The biggest difference between the Mini (seen above on the left) and the cube (the... uh... cube above) isn't the form factor, it's in pricing. Where the regular GamePop is $129 (unless you act soon) the Mini is "free" after a 12-month subscription of $7 per-month, or $84 total. At this price, it costs less than an OUYA, but slightly more than a GameStick. "If you keep it more than 12 months, you keep it forever," BlueStacks' Head of Marketing and Business Development John Gargiulo told us. Of course, there's not much to do with with the Mini without a subscription. "It'd be like if Netflix did it this way and had hardware - the unit would be useless without the subscription." Additionally, if you return the Mini inside of 12 months, there's a $25 restocking fee.

The subscription gives users access to a plethora of games from 500 "popular mobile game partners." Those partners include the teams behind Jetpack Joyride and Fieldrunners. "Getting the kind of developer support we've gotten, it sets us apart. We saw what happened with the Dreamcast and we saw what happened with the Wii U. You need to have good launch titles; there needs to be games everyone recognizes and wants to play."

To make GamePop more enticing to developers, Bluestacks created Looking Glass -- proprietary tech allowing iOS-only apps to run on its Android-4.2-based console. When an iOS app makes calls to Apple's hardware, Looking Glass interprets those calls and translates them to the GamePop Mini's hardware. Of course, a few changes within the code are necessary. "[Porting is] not easy, but I would submit it's not hard, relatively speaking," Garguilo said.

Just yesterday, the Supreme Court of the United States weighed in on two major cases that could affect the rights of the gay community with regards to marriage equality. SCOTUS struck down parts of the Defense of Marriage Act as unconstitutional, and dismissed an appeal by defenders of Prop. 8, thereby clearing the way for same-sex marriages in California to resume. With these decisions setting the stage, this weekend should be the best Pride celebration ever (at least in NYC), and YouTube is here to help usher in the gayness. Here’s what the company said in an official blog post: At YouTube, we?re proud to stand with the LGBT community to support equal rights and marriage equality ? we believe that everyone has the right to love and be loved. Pride Month may be coming to a close, but we hope YouTube is a place where you can feel proud and build a community all year long. 'Dangerous' innovation in legal highs leaves governments in the ...

An explosion of hundreds of new ?legal highs? in recent years has left governments around the world in the dust as lawmakers struggle to keep prohibition laws updated as more and more never-before-seen drugs flood the black market.

The United Nations Office on Drugs and Crime (UNODC) said Wednesday that more than 251 new psychoactive substances were available on the black market by mid-2012, a growth of more than 50 percent over 2009. The UNODC?s 2013 World Drug Report finds that these substances are ?proliferating at an unprecedented rate and posing unforeseen public health consequences.?

The situation has become so severe that the study says the number of new psychoactive substances available on the black market today exceeds the number of controlled psychoactive substances currently prohibited by governments around the world.

One of the problems governments are having with these rapidly emerging substances is that when they place one drug on a schedule of controlled substances, chemists slightly tweak the molecular structure and re-release the drug, once again technically legal thanks to tiny changes.

?There is a lack of long-term data which would provide a much-needed perspective: no sooner is one substance scheduled, than another one replaces it, thus making it difficult to study the long-term impact of a substance on usage and its health effects,? the study notes.

In other words, prohibition laws have now created a game of cat-and-mouse between authorities and the developers of untested new drugs, and the drug developers are winning. A similar effect was observed in the U.S. after authorities banned the psychoactive chemicals in the marijuana-substitute ?Spice,? leading to hundreds of variants of the synthetic drug, some with terrifying physical and psychological side-effects.

The UNODC study said that the global body is now coordinating a rapid-response system that will warn governments of these new drugs as they are discovered on streets around the world.

?We have agreed on a path for our ongoing discussion,? UNODC?s executive director, Yury Fedotov, said in an advisory. ?I hope it will lead to an affirmation of the importance of the international drug control conventions, as well as an acknowledgement that the conventions are humane, human-rights centered and flexible. There must also be a firm emphasis on health and we must support and promote alternative sustainable livelihoods. It is also essential that we recognize the important role played by criminal justice systems in countering the world drug problem and the need for enhanced work against precursor chemicals.?

Hawke and Delpy remain as charming as ever, and their combined goofiness is more endearing than annoying.

Love is messy here, life cannot be controlled, satisfaction is far from guaranteed. Romance is rocky at best. But romance still is.

Though "Before Midnight" is often uncomfortable to watch, it's never less than mesmerizing - and ultimately, a joy to walk with this prickly but fascinating couple again.

"Before Midnight" is heartbreaking, but not because of Jesse and Celine. It's the filmmakers' passions that seem to have cooled.

Before Midnight is fascinating to watch, and so long as Celine and Jesse are communicating, there's still hope.

How (Jesse and Celine) try to rekindle that flame is what drives Midnight, a film that feels so authentic it's like overhearing a conversation you're not sure you should be hearing.

Loving words mix with personal attacks, the magic moments with the unintended slights, as we witness the occasional desperation of imperfect people doing the best they can when life moves beyond meet-cute and courtship. That's authentic.

Linklater and his players bring an end to the fantasy and welcome the thrilling ups and bitter downs of reality to this love story.

Like the first two films, it reflects the real world in a way that seems almost preternatural. It's just that, here, the real world is a harsher, more disappointing place.

The duo, clearly so comfortable in their characters' skin, indulge in intelligent banter, sharp humour and emotional truths.

So much better written than contemporary novels, this film is a literary as well as cinematic achievement to cherish. For grown-ups.

As before, it's often very funny, with Jesse and Celine swapping Woody Allen-esque one-liners - nicely snarky, appealingly abrasive.

The acting, the dialogue and direction are superb.

None of the films is faultless in itself, but, tinted with complementary tones, the complete cycle comes as close to perfection as any trilogy in cinema history.

Marvelous. It's impossible to shake the feeling that we are merely eavesdropping on reality. Witty, wise, and -- most important of all -- truly romantic in ways that movies usually aren't.

It's been 18 years since Hawke, Delpy and Linklater introduced us to Jesse and Celine, and their story just gets richer, funnier and more punchy each time we see them. In 1995's Before Sunrise, they were idealistic 23-year-olds.

Hawke and Delpy are as believably real as any screen couple can ever be.

This is one of the few sequels for which the cliche 'eagerly awaited' is truly applicable.

Predictably, it's just as great as the first two.

By the end, Before Midnight inches towards a dawn of charm. But it's a troubled trip.

As an organic experiment in collaboration between actors and director, it is a triumph, co-created and co-owned by Delpy, Linklater and Hawke.

Hawke and Delpy, who are both credited on the script too, have never found co-stars to bounce off more nimbly or bring out richer nuances in their acting.

The performances and dialogue are wonderfully naturalistic; a reminder that the best special effects are often the cheapest.

Before Midnight is about the nature of long-term relationships, and the way love deepens and grows but also finds itself subject to the complications of maturity. Smart, insightful, and poignant.

For those who witnessed Jesse and Celine's tentative getting together as inter railing students almost twenty years ago, it's reassuring to see them still in love.

Brilliantly directed, superbly written and impeccably acted, this is a thoroughly enjoyable, thought-provoking and emotionally engaging drama that perfectly complements the previous two films.

That's why it's signed a one year marketing deal with Telefonica that'll see the world's fifth largest mobile network pushing WP8 handsets in Europe and Latin America. According to the release, the pair will offer customers Redmond's cloud services like Office 365, Skydrive and "Xbox" in an attempt to coax users away from the 'current duopoly of Android and iOS." Unfortunately there's no details on the specifics of the deal, so we'll guess the pair teamed up purely out of a sense of altruism.

These True Blood "V-80" headphones are actually identical to V-Moda's M-80, which are a durable pair of on-ear cans meant for city life. The sound should be decent, too. Mario Aguilar reviewed its over-ear cousin, the M-100, and liked them just fine. There's a 2500-post strong thread on HeadFi discussing these cans.

The True Blood version comes in a black and red colorway which I think is pretty handsome, at least for a cross-branded product. You also get a little True Blood logo on the cans themselves. But in exchange for advertising HBO's show, you can get these M-80s for $130?a solid $70 less than Best Buy and other retailers, and $10 less than the M-80s when they were the deal of the day a few months back.

Violence kills 27 in minority region of west China

BEIJING (AP) ? Assailants attacked police and other people with knives and set fire to police cars in China's restive far-western region on Wednesday in violence that killed 27 people, one of the bloodiest incidents since unrest in the regional capital killed nearly 200 in 2009.

The early-morning violence ? described by state media as riots ? also left at least three people injured in a remote area of the Turkic-speaking Xinjiang region, the official Xinhua News Agency said. Police stations, a government building and a construction site were targeted in the attacks, it said.

Xinhua said the attackers stabbed victims and set fires, killing 17 people including nine police or security officials, before officers shot and killed 10 of the assailants in Lukqun, a township in Turpan prefecture. The agency cited officials with the region's Communist Party committee.

Xinjiang (shihn-jeeahng) is home to a large population of minority Muslim Uighurs (WEE'-gurs) but is ruled by China's Han ethnic majority. The region borders Central Asia, Afghanistan and Pakistan and has been the scene of numerous violent incidents in recent years, including ethnic riots four years ago in Urumqi, the regional capital.

Xinhua did not provide details about the cause of the unrest and it was impossible to independently confirm the report. Information is tightly controlled in the region, which the Chinese government regards as highly sensitive and where it has imposed a heavy security presence to quell unrest. However, forces are spread thin across the vast territory and the response from authorities is often slow.

The United States said it was closely following the reports of violence, and it urged Chinese authorities to conduct a thorough and transparent investigation and to provide due process and legal protections to those detained.

"We remain deeply concerned by the ongoing reports of discrimination and restrictions against Uighurs and Muslims in China," State Department spokesman Patrick Ventrell told reporters in Washington.

An official reached by phone at the press office of the Xinjiang Public Security Bureau, the region's police, said she had only seen news of the violence on the Internet and had no information. Other officials at the county's propaganda department and police said they also had no details. Calls to the region's government spokeswoman, Hou Hanmin, rang unanswered.

Though it remained unclear what caused Wednesday's violence, police stations, government offices and other symbols of Han Chinese authority have been targets of attacks in the past. The attack occurred at 6 a.m., when most residents would still be asleep.

The report said three assailants were seized, and that police pursued fleeing suspects, though it did not say how many. It said three people were injured by the unrest and were being treated.

The violence came two months after a deadly clash in a town near Kashgar, elsewhere in Xinjiang, killed 21 people, including 15 police officers and community workers.

An overseas Uighur activist said Wednesday's conflict was triggered by the Chinese government's "sustained repression and provocation" of the Uighur community.

Dilxat Raxit, spokesman for the Germany-based World Uyghur Congress, said residents he contacted in neighborhoods about 30 kilometers (18 miles) outside the township said the area has been sealed by "armed forces" and telephone services appeared to be irregular. A heavy security deployment and disruption of communications services also followed the 2009 Urumqi riots.

Dilxat Raxit urged the international community to pressure China to "stop imposing policies in Xinjiang that cause turmoil." Many Uighurs complain that Beijing imposes tight restrictions on their religious and cultural life, barring children and women from attending mosques and discouraging fasting during the Muslim month of Ramadan, which starts this year in early July.

Many Uighurs say they suffer discrimination in jobs and cannot obtain loans and passports.

The Chinese government says all ethnic groups are treated equally and point to billions of dollars in investment that has modernized Xinjiang, a strategically vital region with significant oil and gas deposits. Beijing often accuses overseas Uighur activists of orchestrating violent incidents and obscure militant groups sometimes take responsibility, with little or no evidence to prove claims on either side.

Duncan Innes-Ker, an analyst at Economist Intelligence Unit, said the latest unrest shows that the government needs a new strategy to resolve ethnic and religious tensions in Xinjiang.

"Its past efforts to address them with tight security and economic development have been a manifest failure," Innes-Ker said.

The township of Lukqun is about 250 kilometers (150 miles) southeast of Urumqi along the ancient Silk Road connecting China to Europe.

It is part of an area that includes Turpan, a tourist destination with distinctive Central Asian architecture.

Judge taking no action in Jackson guardianship

(AP) ? A judge says he won't make any changes to the guardianship of Michael Jackson's children after receiving an investigator's report on their well-being and meeting with an attorney for the singer's mother.

Superior Court Judge Mitchell Beckloff ordered the inquiry after Paris Jackson was hospitalized for taking Motrin pills and cutting her arm with a kitchen knife.

The judge said Tuesday he was taking no further action.

Perry Sanders Jr., an attorney for Katherine Jackson, has said Paris Jackson is physically fine and receiving proper medical attention. He has not provided further updates on her condition.

Adding satellite WiFi to an airplane isn't as simple as mounting an antenna up top and flipping the switch on a router -- even installing a cockpit printer requires FAA approval, so as you can expect, the Federal Aviation Administration won't check off on major modifications without some thorough testing. JetBlue's new Fly-Fi service is well on its way to getting a formal green light, though, and is expected to launch before Q3 is through. This week, the carrier is running through a variety of flight tests with one of its Airbus A320s, including maneuvering the plane with some pretty unique weight loads, such as the rear center of gravity positioning you can see demonstrated above. After that's complete, it's time to wait for FAA certification before moving onto performance testing, and if all goes well, passengers should expect to hook up to ViaSat-1 from 30,000 feet in mere months. Once Fly-Fi goes online, it'll be by far the fastest commercial in-flight WiFi option -- we really can't wait!
